01The problem
Pharmacist-led chronic-care programs juggled rosters, care plans, device data, payer eligibility and claims across spreadsheets and separate billing tools.
02My contribution
Developer on the product team, working across the Rails API and the React/TypeScript front end.
03The platform
What the product does. Details are kept generic to respect the client's confidentiality.
- Patient profiles with medications, labs, vitals and immunizations, bulk import and fine-grained privileges per organization.
- Care-program builder with weekly pathways, task queues and questionnaires.
- Continuous glucose monitor data synced every three hours with pre-calculated glucose statistics.
- Encounter notes become CMS-1500 claims with CPT/NDC pricing and electronic submission; HL7 v2 discharge alerts arrive over SFTP.
04Outcome
A long-running healthcare platform of 151 tables, 134 controllers and 72 service objects, with 17 scheduled jobs and integrations into clinical and billing networks.
